    $90 Enrollment fee per adult. Waived if you pay for a year in full upfront.

Medicare LiveActive Chronic Care Management (CCM) Program 

If you have Medicare, we will bill Medicare for our services and the monthly membership is NOT charged. We strongly advise you to participate in our Medicare LiveActive Chronic Care Management (CCM) Program, which is COVERED by Medicare.

The following services are covered under the Medicare Chronic Care Management (CCM) program:

  • Annual Wellness Exam

  • Monthly Chronic Care Visit to assure we are on top of all your chronic health issues

  • Create a Comprehensive Care Plan with each Chronic Care Visit

  • Have 24/7 access to your physician or provider via email, text, or phone call

  • Transition of Care management: We will contact you within 2 business days following a specialist visit or hospital discharge

  • House call* services with coordination with home health and other home services (*within 5 mile radius of our office)​

  • If you reside in a Long Term Care or Independent Living Facility we are happy to serve you there! ​

  • Procedures, labs and imaging are all billed to Medicare

*Cash pricing is also available for medications if you wish to purchase them from our office

Please make sure your Medicare Advisor selects Medicare product(s) for you that include the two following benefits:

  1. CHRONIC CARE MANAGEMENT PROGRAM (CCM) coverage

  2. TELEMEDICINE coverage
