YOUR MEMBERSHIP INCLUDES
Membership rates are based on age (18-39, 40-59, 60+), speak to a representative for more details. For those with Medicare, there is no monthly cost. Medicare is billed directly.

Medicare LiveActive Chronic Care Management (CCM) Program
If you have Medicare, we will bill Medicare for our services and NO monthly membership fee is charged. Your membership is covered via participation in Medicare Chronic Care Management (CCM) program.
The following services are covered under the Medicare Chronic Care Management (CCM) program:
Annual Wellness Exam
Monthly Chronic Care Visit to assure we are on top of all your chronic health issues
Create a Comprehensive Care Plan with each Chronic Care Visit
Have 24/7 access to your physician or provider via email, text, or phone call
Transition of Care management: We will contact you within 2 business days following a specialist visit or hospital discharge
House call* services with coordination with home health and other home services (*within 5 mile radius of our office)
If you reside in a Long Term Care or Independent Living Facility we are happy to serve you there!
Procedures, labs and imaging are all billed to Medicare
*Cash pricing is also available for medications if you wish to purchase them from our office
Please make sure your Medicare Advisor selects Medicare product(s) for you that include the two following benefits:
CHRONIC CARE MANAGEMENT PROGRAM (CCM) coverage
TELEMEDICINE coverage
