Medicare Chronic Care Management (CCM) Program

If you have Medicare, then we want to care for you — regularly. The Chronic Care Management program is structure so we may actively care you, monthly and ongoing.

The following services are covered under the Medicare Chronic Care Management (CCM) program:

  • Annual Wellness Exam

  • Monthly Chronic Care Visit to assure we are on top of all your chronic health issues

  • Create a Comprehensive Care Plan with each Chronic Care Visit

  • Have 24/7 access to your physician or provider via email, text, or phone call

  • Transition of Care management: We will contact you within 2 business days following a specialist visit or hospital discharge

  • House call services with coordination with home health and other home services (within 5 mile radius of our office)​

  • If you reside in a Long Term Care or Independent Living Facility we are happy to serve you there! ​

  • Procedures, labs and imaging are all billed to Medicare

  • Cash pricing is also available for medications through our office

Please make sure your Medicare Advisor selects Medicare product(s) for you that include the two following benefits:

  1. CHRONIC CARE MANAGEMENT PROGRAM (CCM) coverage

  2. TELEMEDICINE coverage
