To request a copy of your medical records or have your records transferred to another healthcare provider, you must complete a Medical Records Release Authorization Form. To access the form, please click here:[ Authorization Form ].

Alternatively, your new healthcare provider may fax a signed medical records authorization form to 727-339-0071 to request the transfer of your medical records.
